Waiver Statement:

Liability Waiver

I understand that by signing this form, I give permission for my child to participate in the Camp Discovery. I understand that reasonable effort will be made to protect and safeguard participants. I understand that when children learn and play there is a possibility of injury. Therefore, I agree not to hold The Powerhouse, its paid staff, volunteers, and partners liable for any illness or mishap occurring during Camp Discovery. This authorization is good while my child is enrolled in The Powerhouse programs or until revoked by me in writing.

Medical Release

I hereby authorize any emergency service agency or physician and dentist associated with the emergency service agency to administer whatever medical care in their professional opinion is necessary for my minor child listed above. The Powerhouse, hospital, and any emergency service agency and their associated physicians, surgeons, and/or dentists have my authority to consult as necessary. This authorization is valid while my child is enrolled in The Powerhouse programs both on-site and off-site, or until revoked by me in writing. I understand that if my child needs to take medication while at the program I will have provided the medication along with the appropriate information and permissions on the liability form. 

Policies and Procedures 

I understand that all participants are expected to follow rules and regulations set by The Powerhouse administrative staff. I understand that all participants will be subject to disciplinary policies and procedures. Staff is prepared to deal with minor behavior issues; however, excessive and disruptive behaviors (e.g.: physically or verbally hurting other children, stealing, property damage, leaving the premises without permission, failure to follow staff directives) will be handled in the following manner: 

First offense: Child will be isolated for a time out and parent will be notified.

Second offense: Child will be sent home and not be allowed to return to camp. No refund given.

I understand that The Powerhouse staff is not responsible for any of the children’s belongings. Children should not bring toys, money, weapons, electronics or other personal items to camp other than what is needed for camp purposes. 

I understand that some parts of the day may require special facilities and will be held away from The Powerhouse campus. In such cases, I give my permissions for my child to be transported to the designated site for his/her activities.

COVID Safety Protocols

Given the evolving nature of the COVID pandemic, our safety protocols are subject to change to best protect our campers and staff. We will determine a mask protocol in early May to reflect the current community health situation. For context, in 2021 we required campers and staff to mask indoors, encouraged social distancing, and performed COVID symptom/contact surveys and touchless temperature checks at check in. If our mask protocol needs to be updated during the summer we guarantee a 3-week notice of any changes. If a camper is showing COVID symptoms, tests positive for COVID, or is exposed to COVID, please do not bring them to camp. We require campers that test positive, show symptoms, or have experienced exposure to follow current CDC quarantine guidelines. Any camps that are missed due to COVD-related quarantines will be refunded on a prorated basis. 

Should a child fall ill during camp, we will isolate them and parents will be immediately notified. Should an educator fall ill, we have backup staff on site ready to step in. If your child tests positive for COVID during camp or within the two weeks of completing the program, notify the education director immediately so that SJBHC and fellow participants in that camp can be notified. The same will be done for a sick educator.

Photography

The Powerhouse takes photographs and/or videotapes of our educational programs. The photographs/videos become property of The Powerhouse and will be used to help promote our programs. I give my permission for my child to be photographed and/or videotaped for the described purpose herein. If I wish to exempt my child from being photographed, I will fill out the appropriate form at check-in. 

Self-Transportation Permission

If indicated on the registration, Camp Discovery participants will be permitted to walk or bike to and from home independently. I acknowledge The Powerhouse cannot be held liable for my child before camp begins or after camp ends.

Late Pickup Policy

Camp Discovery will be open to children from 9:00 a.m. - 3:00 p.m. A $5 per 10 minute fee will be charged when a child is picked up late, beginning at 3:01 pm. 

Refund Policy

Cancelation requests received more than three weeks prior to the start of the session are eligible for an 85% refund. If cancellation occurs within three weeks of the start date, I will receive a 50% refund if and only if the Powerhouse is able to fill my child’s spot in the camp I registered for. 

If The Powerhouse has to suspend activities for any reason or if you need to keep your child home for a documented COVID-related reason, prorated refunds will be given to participants. 

If your child is dismissed from The Powerhouse due to disciplinary issues, no refund will be given.
